Lacombe Registries offers Alberta Health Care Insurance Plan services. You can visit our office to process any of the following:

- New Applications

-Add/Delete a Spouse or Dependants

- Update account address, name etc. (not to be used for updating name due to marriage)

-Request replacement Health Care cards

-Statutory Declaration for Replacement Health Care cards (all of our Registry Agents are Commissioners to help fill out this particular form in full.) 

In addition to these services, Lacombe Registries Ltd. offers lamination services to prolong the life of your card. This can be done upon a walk in request for $2.10/card and takes only a few minutes to complete.
